Assumption Of Risk

Read Before Signing: 

All patrons of Gunnison Sports Outfitters LLC. must complete this waiver form before the beginning of any fishing trip(s).

I hereby acknowledge that participating in fishing activities presents a risk of personal injury, death, and damage or loss of personal property which may result from, but is not limited to the following: fishing and/or wading in or around lakes, ponds, reservoirs or other bodies of water, boarding and deboarding of watercraft, natural hazards including, but not limited to lightning storms and other adverse weather conditions; domestic or wild animals; fire; travel either on foot or in a vehicle in unfamiliar terrain or over fences and other structures, artificial and natural obstructions and unimproved roads, the consumption of provisioned or other food and beverages as well as traveling in company-owned vehicles.

I hereby acknowledge and recognize the existence of these risks, and ASSUME ALL RISKS COMPLETELY.

The use of drugs, including marijuana and all edibles, is illegal in the National Park and is not permitted on any guide trips.  Failure to comply with this law will result in swift and lawful punishment.

I understand that this contract reflects our entire agreement and that my signature indicates that 

I have read and understood this document and that I have signed this document of my own free will.

I understand that this waiver applies to any and all trips I participate in with Gunnison Sports Outfitters, and its owners, agents, officers, and employees. I further state that I am of lawful age to sign this release and waiver agreement or that I am signing it on behalf of a minor for whom I am legally responsible.

Read Before Signing:

I grant permission to Gunnison Sports Outfitters DBA – GSO Fishing, to use my submitted images and/or images (including video) taken of me during the trip, class, or other GSO Fishing event, for use in public media publications including, radio, internet, social media, advertisements, television, print publications, digital blogs, or in any publications, or posts.

I understand that the intended use of such images/video is solely for the purpose of advertising, marketing, fundraising and/or the promotional and public awareness purposes for the organization.

 I hereby waive any right to inspect or approve any and all finished photographs, videos, posts, or advertisements, that may be used in conjunction with them now or in the future, whether that use is known to me or unknown.

I understand that there is no guarantee that any or all of my images/videos (or images of me) will be used. I have been informed that this authorization is voluntary and is subject to revocation at any time, except to the extent that action has been taken in reliance thereon, by notifying the organization in writing at:

I understand that this consent to use images/videos is being made solely for the benefit of Gunnison Sports Outfitters DBA GSO Fishing and comes without any expectation of monetary compensation or other benefits to me. Any benefit accrued or might accrue to Gunnison Sports Outfitters DBA GSO Fishing from the use of images/videos and information, I hereby waive any claim to such benefits.
