Requirements:
You must be a Colorado resident for at least 6 months and you need to have your doctor fill out the one-page disability form. (Downloadable from their website linked below.)
Documents Needed
You must provide one of the following as documentation of a total and permanent disability:
- A fully completed Colorado Parks and Wildlife “Physician’s Affidavit” (located on the application), signed by a licensed physician attesting that the applicant meets the definition of a total and permanent disability; or
- A Division of Workers’ Compensation “Final Admission of Liability” form indicating “Permanent Total Disability”
After completing the application, you can email it to: disability.apps@state.co.us or mail them to
